If you are a retailer interested in selling our products and you would like to receive our wholesale price list/order form, we will first need to verify that you have a state sales and use tax registration number. Please fill out and submit the form below and we will send you an email with a PDF attachment containing our wholesale order form. You will then need to print out the form, fill it out, and fax or mail it back to our offices (the completed form cannot be electronically e mailed back to us). We will not share your information with anyone else.
